Durable Power of Attorney for Health Care
Organizing legal medical documents can be an overwhelming experience for anyone. Fortunately, we can help you complete your Durable Power of Attorney for Health Care. This legal document allows you to name someone to make medical decisions for you if you are unable to make those decisions yourself. To learn more, we invite you attend a FREE, one-hour program at St. Mary's Hospital.
Programs are held every Monday, 12:30 to 1:30 p.m.
Stop by the Information Desk located in our lobby to check on program location. Call St. Mary's Patient and Family Services at (608) 258-6561 if you have questions. You need to be 18 years or older to attend. No reservation necessary.
